Passport Information

Passport Appointments Required
- Appointments for Passport application processing are required.
- A maximum of two applicants per appointment. If there are more than two applicants, we may not be able to complete the additional applications.
- If you are more than fifteen minutes late your appointment is cancelled, and you will need to reschedule.
- Applications are mailed out the following business day.

- If you are using a third-party service, you must have documents showing travel is within 14 days, no exceptions. Please be advised that third party paid for services are not endorsed by the U.S. Department of State and you should make absolute certain you qualify for these services before submitting payment. Consult travel.state.gov regarding urgent travel.
DS-11 application
- Applications may be filled out online and then printed single-sided.
- Applications filled out by hand must be filled out in black ink with no whiteout, strikethroughs, or corrections.
- Do not sign the application until asked to do so by an agent.
- Read the instructions thoroughly and fill each field carefully.
- Before you arrive at the library for your appointment, be certain you have all forms, documents, photographs and forms of payment with you. The library cannot submit incomplete applications, and you may need to schedule another appointment if the agent cannot process your application within your appointment time slot.
- The U.S. passport card can be used to enter the United States from Canada, Mexico, some Caribbean countries, and Bermuda at land border crossings or seaports-of-entry. The Passport Card cannot be used for international travel by air. View for more information.
Minors and 16/17 Year Olds
- Applicants who are 16 or 17 years old may apply for an adult passport which will be valid for ten years. There are instructions specific to these applicants. Please consult Apply for a Passport as a 16-17 Year Old webpage for the requirements.
- Applicants under the age of 16 may apply for a child’s passport. Passports for children under age 16 are valid for five years. If a child’s passport has expired and they are under 16 years of age, they need to apply for a new passport using the DS-11 form. There are instructions specific to first time child applicants and those who passports are expired. Please consult the Apply for a Child’s Passport Under 16 webpage for requirements.
Evidence of U.S. Citizenship
- U.S. birth certificate that meets the following requirements: Shows the seal or stampfrom the city, county, or state of birth which issued it; the applicant’s full name, date of birth, place of birth; your parent(s)' full names; the date filed with registrar's office (must be within one year of birth); and the registrar's signature.
Or
- Full validity, undamaged U.S. passport
- Full validity means the passport was valid for 10 years for adults and 5 years for children under 16.
- Consular Report of Birth Abroad or any birth document issued by the U.S. Department of State
- Certificate of Naturalization or Citizenship
- If you have an Alien Registration Number (A-Number) from USCIS and you are applying for the first time, you may include the A-Number on your passport application.
- The U.S. Department of State does not accept digital documents, all documents presented must be physical documents.
Passport Photograph
- All applicants must submit a photograph with their application.
- We strongly recommend you consult the Passport Photo webpage as the requirements are stringent. If the Passport Agent accepting your application feels that your photograph will not meet the U.S. Department of State requirements, the agent will recommend you submit a different photograph.

Proof of Identity
- If the applicant is a minor under the age of 16, the parents/guardian are responsible for providing proof of identity. If the applicant is age 18 or older, the applicant is responsible for providing proof of their identity. Consult the Photo Identification webpage for acceptable documentation. The documentation will need to be presented to the Passport Agent at your appointment.
- Applicants age 16-17 must present Photo Identification. Additionally, if one parent/guardian is with the applicant, they should also provide proof of identity.
Payment
- The U.S. Department of State requires payment by personal check, certified check or money order only. Please consult the Passport Fees webpage.
- The U.S. Department of State requires the library as an Acceptance Facility to charge a $35 processing fee for each application. Payment may be paid using check, credit card or cash (please bring exact amount of cash). Additionally, if you opt to send your application via Priority Mail Express, there is an additional fee due and payable to the library. Priority Mail Express rates are set by U.S. Postal Service and fluctuate, please contact the library, passports@tclib.org for current rates. Applications are otherwise mailed via Priority Mail, and the library is not allowed to transfer this cost to the applicant.
Other Information
- Current Processing Times fluctuate, please consult Get Your Processing Time for current information. Per the U.S. Department of State, “Processing times only include the time your application is at one of the passport agencies or centers. The total time to get your passport includes both processing and mailing times. It may take up to 2 weeks for applications to arrive by mail at a passport agency or center, and up to 2 weeks for you to receive a completed passport in the mail after we print it. Consider the total time it will take to receive your passport when you are booking travel.”
