Drop in to meet with a Community Health Outreach Worker!
Every Friday
2:00-4:30 p.m.
Study Room 5
Do you have medical questions but aren't sure who to ask? Drop in to the library during CHOW hours and speak to a Community Health Outreach Worker from Teton County Public Health.
The CHOW Program connects community members and all the different medical/social services they have access to (but may not know about). They can help you find medical and behavioral health professionals, navigate the health insurance system, and locate vital resources such as transportation, food, and financial aid. They can also provide informal counseling, health screenings, and referrals as well as education on topics related to chronic disease, prevention, physical activity, and nutrition.
Help is available in English and Spanish.
For more information, contact Odalis at (307) 732-8460. This service is provided by Teton County Health Department.
